Do I need a referral?
It is beneficial to obtain a referral from your General Dentist, Doctor or other Specialist. It is required in order for you to receive a Medicare rebate.

What do I need to bring to my consultation?

• Current referral
• OPG x-ray (plus any others you may have been given)
• Medicare Card, Private Health Insurance details, DVA card (if applicable)
• List of any medications you are currently taking
• Form of payment (EFT, MasterCard or Visa accepted)
• Diary with work rosters or commitments
